District 202 Health Services welcomes you and your child as he/she begins elementary school!
The Illinois School Code Sec 27-8 requires all incoming Kindergarteners to have a health/physical examination with the required immunizations completed prior to the first day of school.
Incoming kindergarteners must use the State of Illinois Certificate of Child Health Examination form.
Lead Screening Requirement: Six years of age and under
Now through June 2026, students age 6 and under who live in the following zip codes are required to have a blood lead test completed as part of the physical examination: 60432, 60433, 60434, 60435, 60436, 60544, 60586.
Beginning July 1, 2026, Illinois law requires ALL students age 6 and under to have a blood lead test on file as part of their school health requirements. Please note that physical examinations will be considered incomplete if the required blood lead test is not completed and submitted with the physical examination.
